How to Open a CCC Apply Account
K-12 Dynamic Form Student Supplemental Application
This form is required for a student who wishes to enroll in a Dual Enrollment class. As a student, once you have applied to LACC, you can fill out this from from your student portal. The form must be eletronically signed by the student, their parent or legal guardian, and the school counselor. It should include the student's College ID number.
