Your educational needs are
our primary concern. In order to meet those
needs, we have developed this contract, which
outlines the program’s responsibilities and your
responsibilities.
By adhering to this contract, you will be
actively participating in meeting your
educational and career goals.
Within its resources, the
Student Support Service program will provide you
with the following services:
Career Exploration
Financial Aid Information
Transfer Information
Professional Counseling
Cultural Events/Field Trips/Workshops
Student Educational Plan
Educational Workshops
Tutoring Referrals
Scholarship Information
Computer Lab Access
As a recipient of the
Student Support Service program, I understand
and agree to do the following:
1.Enroll in nine (9) or more units each
semester and maintain at least a 2.0 cumulative
grade point average.
2.Meet with a SSS program counselor at
least once per semester to develop and update
the educational plan.
3.Follow an educational plan as
prescribed. Enroll in remedial or prerequisite
classes as directed.
4.Before adding or dropping a class, it is
highly recommended that you see a SSS program
counselor.
5.Attend at least two workshops
and/or cultural event/fieldtrip each semester.
6.See a SSS program counselor immediately
if having difficulties in any class (es).
7.Notify the Admission & Records office and
the SSS office of change of name, address, or
phone number, and social security
number.
I realize that failure to
comply with the above responsibilities may
affect my status in the Student Support Service
Program.
