How to Apply: New Students

Students new to OSS (Office of Special Services) are REQUIRED to complete the OSS application process. The application process requires four (4) items:

  1. Download and complete the New Student Application Form using Adobe Acrobat Reader.
  2. Read through and complete the OSS Orientation and Orientation Quiz. A link to the quiz can be downloaded at the bottom of the OSS Orientation page
  3. Verification of your disability. Students with and Individualized Education Plan (IEP) and/or 504 students can submit these documents. Or, a letter from a medical professional or learning disability specialist who can verify your disability.
  4. Complete the Voter Preference Form Visit the CA Secretary of State site to download the Voter Preference Form in your preferred language.
